A fast growing Home Health company in Ventura County is currently accepting applicants for the position of Intake Coordinator/Receptionist.
Responsibilities:
• Receives, coordinates and tracks daily patient referrals from hospitals. Clinics, facilities and from other sources;
• Verifies Medicare and Medical eligibilities of all patient referrals;
• Coordinates initial staffing of accepted patients
• Coordinates with referral sources regarding patient status and communicates with family member, direct care staff, and case management;
• Ensure compliance with all state, federal and JCAHO referral/intake regulatory requirements;
• Answers telephone and relay messages appropriately as required;
• Assist with HR
• Performs other duties as assigned.
Qualifications:
• College Level;
• Excellent and technology skills;
• Possess positive attitude and excellent communication and customer service skills;
• Organized and able to multi task meticulous documentation;
• Goal oriented , proactive and takes initiative;
• Must work independently in a team environment;
• Medical Background is strongly preferred.
